Adding and subtracting from this 'Quick Mask' is
simply a matter of using several tools. I used the 'Airbrush'
extensively here. I zoomed in tight and selected a very small
brush (about 3 px size). Now using my Wacom pen I started fine
tuning my mask. If you select white as a color you subtract from
the mask, black adds to the mask. Don't forget to toggle back
and forth between 'edit mode' and 'quick mask mode' (toggle with
the 'Q' key). It helps you see what areas you are missing.

Other tools you can make use of are the 'Dodge'
tool (this lightens an area) and the 'Burn' tool (this darkens
an area). Continuing add and subtracting from the quick mask
until you have a very detailed mask. Now I add a 'Gaussian Blur'
effect to the 'Quick Mask.' I try different settings and check
them using the 'Preview' check box. In this case I decided on
.5. This is just enough to blur the edge of the mask.
